Applicants for screening are required to submit the following items:

• Previous year's Income Tax Statement (not just W2 form)
  
  
• Two most current check stubs;
  If more than one employer in current year, include final check stub of each.
  
  
• Driver's License / Photo ID
  
  
• Proof of residency (anything addressed to your home)
